Memberships Terms & Conditions 

• Commitment Period: This membership requires a commitment for 6 consecutive months. After the initial sign-up, the membership will continue on a month-to-month basis until canceled by the member. 
• Membership Fee: The monthly membership fee is based on which tier is chosen. We require a card on file that will be charged immediately upon sign-up. Subsequent payments will be charged on the first day of each month. 

• Cancellation Policy: To cancel your membership, you must provide Skintonomy Medical Spa with a minimum of 30 days written notice before the next billing cycle. Membership cancellations require written documentation. Please ask our staff for a cancellation form. Documentation can be provided in person or via email. 
• Refund Policy: Membership fees are non-refundable and will not rollover. Refunds will only be granted in the event of an error. 
• Membership Benefits: As a Skin Med babe, you will receive the benefits of the respective membership tier you signed-up for. The benefits are outlined on our website and are subject to change at any time. 
• Membership Termination: Skintonomy Medical Spa reserves the right to terminate your membership at any time for any reason at our discretion. 
• Personal information: By signing up for a Skin Med membership, you agree to provide accurate and up-to-date personal information, including name, address, payment information, and relevant health diagnoses. We will use this information solely for the purpose of managing your membership.

Patient Agreement: By signing a Skin Med Membership Agreement Form, you must acknowledge that you have read and agree to the terms and conditions.
